- H.S.C. or equivalent certificate, in support of declaration of age
- B.V.Sc. & A.H. Degree Certificate issued by the recognized University/College/Institution
- Mark-lists in support of all examinations from H.S.C. to Degree in B.V.Sc. & A.H., including fail marks, if any
- Conversion Certificates recognized by the Veterinary Council of India (VCI), for candidates with foreign degrees
- Conversion Certificate from the concerned University, for candidates awarded only grade marks instead of percentage
- Permanent Registration Certificate under the Odisha Veterinary Practitioners Act, 1970
